Two parts to that answer.
During the Cold War, it was very much an either/or situation. You were either West, or East. Even nominally neutral countries were in either camp; there was simply no room to choose an independent course. Sweden and Finland were West; North Korea and Yugoslavia were East.
After the Cold War, the military-industrial complex in the US had to find new enemies to motivate continued military spending. They coined the phrase “rogue states”, and filled it with content. By now, they have so much affected the US mentality that many Americans find it incomprehensible not to have any enemies to fight; you can see, for instance, how then Speaker Newt Gingrich said that the main lesson that the US had learned during the 20th century was that their foreign policy should be focused on “identifying and neutralizing our enemies”. Of course, if you go out and try to find enemies, you will find no shortage. Finding friends was not a concept he could understand.
And yes, “rogue states” is a concept that you will hardly ever find discussed outside the US.
