He should have used Al's surname and introduced him first. The manager is the more important person, which is why his name would be the first mentioned. Jim also did not introduce John as his brother, so Al has no idea who or why he is being introduced. 

I think they were surprised. I've read the book before in my class and to me personally they were surprised. I hope this helps
