A. Rate of change is 50 and initial value is 1200, you find this by knowing that when he started his business (2005) he was charging 1200$ and in a period of three years that value increased by 150$ (150/3=50 the rate of change) 
y= 50x+1200 is the slope intercept form

B. y=50x+1200
